Tooth System

On the standard ADA claim form, there is a column labeled "Tooth System."

  • In the U.S., the ADA's Universal/National Tooth Designation System is most commonly used (1 - 32 for permanent teeth; A - T for primary teeth). This is referred to as the "JP" system, and thus you should enter "JP" on the claim form.
  • When using the International Standards Organization System, enter "JO". See the Specification 3950 page for details.
